The State of Illinois requires immunizations, physical examinations, eye and dental exams for children in school.  

Now is the time to schedule those appointments to avoid your child missing opportunities or being excluded from school.

Please review this information which is organized by grade-level.  Failure to provide proof of meeting these requirements by October 15 will require us to exclude your child from school until they are in compliance.  

If you have trouble locating a provider or need other assistance,  please contact your child's school to reach a school nurse for help.
